Radiologic Biomarkers of Acute Non-Ketotic Hyperglycemic Chorea: A Systematic Review

Objective: To systematically review the case reports and case series of Acute Non-Ketotic Hyperglycemic Chorea (ANKHC) patients with signal changes in MRI.

Background: Acute and subacute chorea are considered movement disorders, although they are generally secondary to a myriad of insults – vascular, metabolic, infectious, inflammatory, hypoxic, traumatic, chemical. ANKHC is a well-recognized hyperkinetic syndrome in type 2 Diabetes mellitus patients. Hyperintensity in at least one basal ganglia contralateral to the involved side in the T1 magnetic resonance imaging (MRI) is considered the hallmark sign of this entity since it is presented in nearly all cases, in the appropriate clinical context. Various metabolic and structural hypothesis for this phenomena were proposed, but the exact pathophysiology remains inconclusive.

Methods: We have conducted a systematic review of case reports and series of cases who presented typical signal changes of ANKHC in MRI and study their localization within basal ganglia. The protocol will follow PRIMA-P guidelines (Shamseer, Moher et al. 2015) and will be registered at Prospero database (CRD42015026397). The methods of this systematic review and meta-analysis will follow PRISMA (Liberati, Altman et al. 2009) guidelines. Reporting of statistical data will follow SAMPL (Lang and Altman 2013) guidelines.

Results: The definitive results of our research will be presented at the event.

Conclusions: Data from this review will provide insight on the definition of the anatomy of ANKHC, in the pursuit of a pathognomonic imaging finding in the context of hyperglycemic chorea.
