Who uses guidance techniques for abobotulinumtoxinA injections in the routine management of cervical dystonia?

Objective: Describe the use of injection guidance techniques in different cervical dystonia (CD) patient populations treated in routine practice.

Background: The use of injection guidance techniques is often recommended by experts to improve accuracy of muscle targeting and effectiveness of botulinum toxin (BoNT) injections.

Methods: The MetaCD database includes data from 2 observational international studies (INTEREST IN CD1 [NCT00833196)] & INTEREST IN CD2 [NCT01753349]) and 1 USA registry (ANCHOR-CD [NCT01314365]), conducted between 2009 and 2017. Here, we describe the use of injection guidance (at baseline/first injection visit and at 1 year) in 585 subjects routinely treated with abobotulinumtoxinA (aboBoNT-A) over 1 year. Subjects were categorized by previous exposure to BoNT treatment (previously treated and naïve) and by region (Europe, USA and rest of world [ROW]).

Results: Use of injection guidance was consistently higher in subjects who were new to BoNT treatment versus those who had been previously treated [table1]. Consistent regional differences in use of guidance were also noted USA>ROW>Europe. The use of injection guidance remained relatively stable over 1 year of follow-up, even in those subjects who were naïve to treatment at baseline but who had generally had undergone a series of repeat treatments by the 1 year visit.

Conclusions: These results show that injection guidance is used in less than half of routine aboBoNT-A injections for CD, and is more commonly used in the USA than other regions. Regional differences may be due to the costs of acquiring guidance equipment or reimbursement incentives. The higher use of guidance techniques in new patients could indicate a trend towards greater utilization.
