Parkinson`s disease in Pandemic Covid

Objective: SARS-CoV2 infection invades the brain through ACE2 receptors or the cytotoxic aggregation of proteins, including α-synuclein.

Background: Patient G. I., aged 75, is brought urgently to the Brașov County Emergency Clinical Hospital for: altered respiratory and neurological condition. IOT is decided in UPU and transferred to ICU COVID.
Hospitalization diagnosis: Acute respiratory failure IOT-VM, 6-point GCS, SARS-COV2 infection, Hypertension, Parkinson’s disease, sequelae stroke , mixed dementia, pulmonary fibrosis, metabolic and respiratory acidosis, hypercapnia.
Patient with severe general condition, IOT-VM, hemodynamically stable, continues to monitor vital functions, hydroelectrolytic and acid-base rebalancing therapy. Treatment is initiated according to the protocol, also associated with chronic treatment. The evolution is favorable, on day 4 it is decided to extubate and continue the oxygen therapy on the facial mask. On the 11th he is transferred to the Neurology Department for monitoring and further treatment, and on the 17th he is discharged at home.

Method: case file

Results: The combination of Parkinson’s disease and SARS-COV2 infection has a very poor prognosis, especially in patients with IOT and severe ARDS. In our clinic he is the second patient of this kind since the beginning of the pandemic, extubated after several days, transferred, arrived home.

Conclusion: Parkinson’s disease is an independent risk factor that involves more care, but should not condemn the patient to SARS-CoV infection.
