The effect of lumboperitoneal shunt surgery on patients with iNPH plus Parkinson’s disease

Objective: To investigate the effect of lumboperitoneal shunt surgery on patients with iNPH comorbid with Parkinson’s disease

Background: Comorbid Parkinson’s disease (PD) in idiopathic normal pressure hydrocephalus (iNPH) affects the clinical features and disease course. MIBG myocardial scintigraphy and DaT scan are useful for detecting comorbid PD. Importantly, our retrospective analysis showed that lumboperitoneal shunt (LPS) surgery improved the short and long-term disease courses, including life span. In this study, we tried to confirm the beneficial effects of LPS surgery on iNPHc+PD by the anterospective, multi-site, randomized controlled study.

Method: From May 2021, according to the criteria, we started to recruit patients with PD comorbid with iNPH (iNPHc+PD), who met both PD and iNPH criteria. They were randomly divided into two groups. In Group 1, LPS surgery was conducted immediately after establishing the diagnosis, and in Group 2, LPS surgery was conducted after three months. We conducted a clinical assessment using our test battery and brain MRI and DaT scan at certain time points.

Results: We have recruited 36 patients with iNPHc+PD till March 15, 2022. Sixteen patients were included in Gpoup1, and 20 patients were included in Gpoup2. The gender, average age, and scores of Hoehn & Yahr and MMSE were not statistically different between groups. No significant surgery-related abnormal effects have been observed.

Conclusion: We have succeeded in starting the study. We will keep recruiting participants till May 2023.
