Objective: To present the draft International Parkinson and Movement Disorder Society (MDS) guidance framework for validating, evaluating, and implementing electronic clinical outcome assessments (eCOAs).
Background: As COAs transition from paper to electronic formats, harmonized standards are needed to ensure scientific rigor, regulatory alignment, and patient-centered usability. Existing MDS guidance has focused mainly on patient-reported outcomes (PROs) and clinician-reported outcomes (ClinROs), leaving limited direction for electronic observer-reported outcomes (ObsROs) and performance outcomes (PerfOs). This framework addresses these gaps by outlining best practices for establishing equivalence and usability across eCOA types.
Method: The MDS Electronic COA Committee and COA Scientific Evaluation Committee conducted a review of current MDS guidance, global regulatory expectations, and published evidence on eCOA migration. A gap analysis informed updated recommendations, refined through iterative committee review.
Results: The draft framework provides methodological standards for electronic migration, cognitive debriefing, usability testing, and psychometric evaluation across PROs, ClinROs, ObsROs, and PerfOs. Key enhancements include: (1) a tiered evidence structure for demonstrating equivalence with paper formats; (2) standardized criteria for user interface design, accessibility, and device variability; and (3) guidance for remote and hybrid assessment models.
Conclusion: The MDS draft eCOA framework offers a unified, rigorous approach for developing and validating electronic assessments in movement disorders research. By clarifying evidence and usability expectations, it supports stronger scientific standards, improved regulatory readiness, and broader adoption of high‑quality eCOAs. Next steps include internal MDS review, external expert validation, publication as official position papers, and integration into the MDS COA review process.
