Objective: To evaluate whether benign prostatic hyperplasia (BPH) is associated with incident Parkinson’s disease (PD) in men and to explore whether BPH-related medications may influence this association.
Background: BPH and PD are common age-related disorders, but prospective population-based evidence linking BPH to later PD remains limited.
Method: We analyzed 182,927 male participants in UK Biobank who were free of PD and major neurodegenerative disease at baseline. Baseline BPH was identified from first-occurrence records, and incident PD was ascertained using ICD-10 code G20 through November 30, 2022. Cox proportional hazards models were used to estimate hazard ratios (HRs) and 95% confidence intervals (CIs) after adjustment for demographic, socioeconomic, lifestyle, clinical, and genetic factors. Propensity score matching and multiple sensitivity analyses were performed.
Results: During approximately 13 years of follow-up, 1,704 incident PD cases were identified. Baseline BPH was associated with a higher risk of incident PD in the fully adjusted model (HR 1.23, 95% CI 1.06–1.42). This association persisted in the propensity score-matched cohort (HR 1.30, 95% CI 1.09–1.55) and in sensitivity analyses, including time-varying exposure analysis (HR 1.28, 95% CI 1.14–1.44) and exclusion of PD events occurring within the first 2 to 8 years of follow-up. In medication-stratified analyses, the excess risk appeared attenuated among participants receiving alpha1-adrenergic receptor blockers (HR 1.09, 95% CI 0.82–1.45).
Conclusion: BPH was associated with an increased risk of incident PD in this large prospective cohort. Further studies are needed to clarify underlying mechanisms and the potential role of BPH-related treatments.
