Implementation of a Hindi REDCap Instrument for the Unified Parkinson’s Disease Rating Scale (UPDRS)

Objective: To develop a Hindi-language electronic instrument of the Unified Parkinson’s Disease Rating Scale (UPDRS) within the REDCap system, ensuring accurate representation of the validated Hindi translation while enabling standardized multilingual data collection for clinical and research use in Parkinson’s disease.

Background: The UPDRS, developed by the Movement Disorder Society, is widely used to assess the severity and progression of Parkinson’s disease. Although validated translations exist in several languages, including Hindi, standardized electronic implementations remain limited. Integrating multilingual clinical instruments into digital platforms can improve accessibility, streamline data collection, and facilitate multicenter research. This project aimed to implement a Hindi-language UPDRS instrument within REDCap.

Method: The validated Hindi translation of the UPDRS was converted into Unicode format to ensure compatibility with REDCap and consistent display across systems. The instrument was built within REDCap following the original UPDRS structure and scoring format. The Multi-Language Management module was used to integrate the Hindi version alongside the base instrument, allowing seamless switching between languages. Field types, response options, and scoring structures were configured to mirror the standard UPDRS format. Screenshots of the REDCap instrument were reviewed by another team member to verify fidelity to the original Hindi translation and ensure accurate rendering and layout.

Results: A fully functional Hindi-language REDCap instrument for the UPDRS was successfully implemented. The instrument retains the original scale structure and response options while enabling assessments to be completed in Hindi through REDCap’s multilingual interface. Unicode encoding ensured consistent display of Hindi characters across devices and browsers. The electronic format facilitates standardized data entry and supports efficient data management for clinical and research applications.

Conclusion: Implementing a Hindi UPDRS instrument in REDCap provides a practical solution for multilingual electronic data capture in Parkinson’s disease research and clinical practice. This approach enables standardized use of the Hindi scale while leveraging REDCap’s secure data management capabilities, supporting broader inclusion of Hindi-speaking populations in movement disorder research.
