Endovascular therapy in acute ischemic stroke(AIS)

Objective: The objective of the study was to identify the frequency of subarachnoid hemorrhage(SAH) in patients with AIS, treated with intra-arterial thrombolysis(IAT), mechanical thrombectomy(MT) and endovascular recanalization techniques.

Background: Endovascular intracranial recanalization may be complicated with SAH. Parenchymal hematoma and hemorrhagic infarction can also be present in revascularization treatment.

Method: The study is a retrospective analysis. Patient selection was from January 2016 through August 2018. Endovascular treatment includes IAT, thrombectomy and/or angioplasty with and without stenting. CT and MRI were reviewed to identify SAH. National Institutes Of Health Stroke Scale(NIHSS) score was used 3 hours after intervention.

Results: A total of 161 patients were enrolled. Primary mechanical trombectomy was used for 129 patients and 32 were treated with IAT. SAH appeared in 22 patients(12.6%). Isolated SAH occurs in 9 patients and both SAH and intracerebral hemorrhage(ICH) at 10 patients. Only 3 presented ICH.

Conclusion: SAH is one of the most several complication in endovascular AIS therapy. The frequency of SAH was numerically but not significantly higher in patients treated with primary IAT. A larger cohort is needed todetermine these incidences are true.
