Gender effects on clinical features in LRRK2 G2385R carriers and non-carriers in Parkinson’s disease

Objective: This study was to explore gender effects within the same genetic subtype and role of leucine-rich repeat kinase 2 (LRRK2) G2385R variants within the same gender.

Background: Differences between genders have been studied for PD previously. Mutations in the leucine-rich repeat kinase 2 (LRRK2) gene are frequent genetic cause in PD and the polymorphic variants G2385R are the most common variant in Asians. Nonetheless limited data are available regarding gender effect with respect to LRRK2 G2385R variants as well as effects of LRRK2 G2385R variants in terms of gender.

Methods: Patients were recruitment from Movement Disorders Clinic. Clinical data including demographic information, disease history, scales of motor and non-motor symptoms, midbrain transcranial sonography and DNA was collected. Binary logistic regression analysis was performed to evaluate the possible association between the clinical features and gender in LRRK2 G2385R carriers and non-carriers, and to evaluate the possible association between the clinical features and LRRK2 G2385R variants in male and female.

Results: 410 PD patients were enrolled. Gender distribution is similar in LRRK2 G2385R carriers and non-carriers (p=0.738). Among male gender, LRRK2 G2385R carriers showed lower frequency in cognitive impairment (OR=0.286, p=0.003) while there is no significant association for dementia (OR=0.973, p=0.916) in female for LRRK2 G2385R variants. Among LRRK2 G2385R non-carriers, female showed milder impairment in activity of daily living (ADL) (OR=0.531, P=0.011), lower excessive daytime sleepiness (EDS) frequency (OR=0.531, P=0.011), lower substantia nigra (SN) hyperechogenicity frequency (OR=0.448, P=0.019), lower autonomic dysfunction frequency (OR=0.570, p=0.019) and higher mood disorders frequency (OR=1.848, p=0.022). In LRRK2 G2385R carriers, no variable was shown to be significant different between genders.

Conclusions: A more benign disease course was observed in female compared to male in LRRK2 G2385R non-carriers. Gender differences were less notable in PD with LRRK2 carriers.
