Clinical Characteristics of patients with Parkinson’s disease showing normal values on SBR in the DAT SPECT

Objective: The aim of this study was to clarify what the ‘normal’ values of the specific binding ratio (SBR), a measure of striatal radiotracer uptake estimating amount of dopamine neurons quantitatively on dopamine transporter single-photon emission computed tomography (DAT SPECT), suggests on the features of the case at the diagnosis of Parkinson’s disease.

Background: Parkinson’s disease (PD) is neurodegenerative disorder affecting striatonigral system. Degeneration of dopaminergic neuros in the striatum is estimated by the DAT SPECT, there are, however, a large differences of the SBR values at the diagnosis of the cases. We studied the clinical features and courses of the cases showing ‘normal’ value of SBR.

Method: We analyzed the SBR in DAT SPECT examination from 144 patients with PD according the diagnosis criteria of MDS. The clinical features of patients, responses to the treatment and SBR in DAT SPECT were studied.

Results: The estimated SBR at the diagnosis of Parkinson’s disease in the predominantly affected side was more than 5 point which is ‘normal’ on control cases in 11 cases of 144 patients with Parkinson’s disease. Among the 11 cases, neurological examinations revealed 3 tremor dominant cases, 10 cases showing good response to treatments and 10 cases with no depression or anxiety.

Conclusion: Our results suggest that the cases showing ‘normal’ values of SBR in DAT SPECT analysis at the diagnosis of PD developed benign course of the disorders. ‘Normal’ SBR in DAT SPECT is an pathophysiological marker to know the course of the disease.
