Objective: To obtain knowledge that would lead to the usage of local field potentials (LFP) in the deep brain stimulation therapy (DBS).
Background: The LFP of patients with Parkinson’s disease (PD) is not fully understood, and its usage in the DBS is still in the early stages.
Method: The LFP pre- and post- stimulation were evaluated in 4 patients with PD (Male 1: Female 3), with median age of 63.5 years [Interquartile range: IQR 55.5, 67.7] and a disease duration of 7.0 years [6, 12.5], who underwent bilateral subthalamic nucleus (STN) DBS.
Results: {Pre-surgery} The Movement Disorder Society Unified Parkinson’s Disease Rating Scale motor score in the off-medication state (i.e., no medications for at least 12 hours) was 21.0 points [18.5, 43.7], and the daily total dosage of antiparkinsonian drugs (Levodopa Equivalent Dose: LED) was 750.0 mg/day [567, 1186]. {Post-Surgery, Pre-Stimulation} The motor score in the off-medication/off-stimulation was 23.5 points [16.7, 29.5], and the LED 550.0 mg/day [500, 1048]. The opposite side STN LFP to the side with predominant motor symptoms had a frequency of 16.6 Hz [15.1, 18.7], and an intensity (Least significant bits) 1651.0 [307, 6104]. {Post-Stimulation} The motor score in the off-medication/on-stimulation was 10.0 points [5.7, 21], and the LED was 300.0 mg/day [150, 898]. The stimulation parameters were an amplitude of 1.1 mA [1, 1.5], a pulse width of 60 μs, and a pulse frequency of 125Hz. Except for one case, LFP was inhibited (LFP-intensity 325.4 [181, 2367]).
Conclusion: After stimulation, the parkinsonian symptoms improved, and the dosage of drugs was reduced. Additionally, the amplitude of LFP decreased in all but one case. In the one case, DBS did not suppress LFP, and it is possible that the attenuation of the microlesion effect was insufficient. To effectively utilize DBS, it is necessary to examine LFP in a larger number of cases.
