Unsupervised multimodal digital assessment of fatigue and sleep disorders in Parkinson’s disease: the IDEA-FAST study

Objective: To evaluate fatigue and sleep disorders in unsupervised conditions in neurodegenerative and autoimmune conditions using a combination of mobility, autonomic and sleep digital home assessment.

Background: The standard evaluation of non- motor symptoms relies on self-reported questionnaires, and objective measure of these symptoms are still lacking. Digital technologies have the potential to measure and monitor fatigue and sleep disorders in real life.

Method: The overarching aims of IDEA-FAST (“Identifying Digital Endpoints to Assess FAtigue, Sleep and acTivities of daily living in Neurodegenerative disorders and Immune-mediated inflammatory diseases”) are to: 1) provide a digital solution able to quantify fatigue and sleep disorders across different disorders including PD, Huntington’s disease, Rheumatoid Arthritis, Lupus erythematosus and Inflammatory Bowel Disease; 2) validate digital outcomes in predicting clinical changes at different time points across the different disease entities 3) obtain regulatory and health stakeholder approval for fatigue and sleep assessment outcome measures derived from home-based digital technology.

Results: IDEA-FAST is a multi-center project funded by the European Innovative Medicines Initiative. It includes 47 partners from leading international universities and some of the world’s largest pharmaceutical and technical companies. The project began in November 2019 and will run for five years.  The focus is the assessment of fatigue and sleep disorders across neurodegenerative and immuno-mediated disorders with (a) digital outcome measure(s). A total of 2000 participants are to be included in the study, 500 of whom will have PD. At the time of submission, 400 / 150 participants have already been enrolled. The feedback from the study participants on the study design is basically positive, there is great interest in the novel technology and the options arising from it.

Conclusion: The IDEA-FAST study will evaluate fatigue and sleep disorders using digital devices in real-life scenario across different neurodegenerative and autoimmune disorders. The digital assessment will allow a deeper understanding of fatigue and sleep disorders and thus potentially enable the testing of new intervention strategies modulating these important symptoms in different disorders.
