Objective: To assess the feasibility, acceptability, and preliminary outcomes of an online fatigue self management programme for people with Parkinsons disease.
Background: Fatigue is a highly prevalent and disabling non motor symptom of Parkinsons disease, with limited evidence supporting non pharmacological management strategies. While fatigue self management interventions have demonstrated benefit in other neurological conditions, their feasibility and acceptability in Parkinsons disease remain insufficiently explored.
Method: A pilot parallel group randomised controlled trial was conducted comparing an online fatigue self management programme with usual care. Adults with Parkinsons disease experiencing fatigue were recruited remotely and randomised to intervention or waitlist control. The programme was delivered over six weeks and adapted from an established fatigue self management intervention. Feasibility outcomes included recruitment, retention, and adherence. Secondary outcomes included fatigue severity, fatigue self efficacy, sleep quality, mood, and occupational performance, assessed at baseline and post intervention. Analyses were descriptive and exploratory.
Results: A total of 155 participants consented to participate, with 98 completing baseline assessments. Retention at post intervention was 60 percent. Adherence varied across programme modules, with higher completion rates observed in earlier sessions. Participants in the intervention group demonstrated improvements in fatigue self efficacy and occupational performance, with smaller and more variable changes observed in fatigue severity and secondary outcomes. No adverse events related to the intervention were reported.
Conclusion: Online fatigue self management appears feasible and acceptable for people with Parkinsons disease and provides preliminary signals of benefit, particularly in fatigue self efficacy and daily functioning. Engagement and retention challenges highlight the need for further optimisation prior to a fully powered randomised controlled trial.
