Objective: To evaluate value driven outcomes of an integrated multidisciplinary Parkinson’s clinic for older adults in Singapore
Background: Parkinson’s Disease (PD) is a complex neurodegenerative condition requiring coordinated multidisciplinary care [1,2]. The Integrated Parkinson’s Hub at Alexandra Hospital, Singapore provides comprehensive management for older adults with Parkinsonism through a team comprising a geriatrician, neurologist, geriatric nurse, pharmacist, physiotherapists, and care managers.
Method: We retrospectively reviewed all first visits to the Hub between January 2022 and December 2024. Seven clinical indicators, aligned with international PD care standards, were assessed, including healthcare utilization, early physiotherapy review, autonomic assessment, avoidance of dopamine-blocking agents and care consolidation. A Composite Quality Index (CQI) was calculated as a composite of these indicators. Mean cost per patient was calculated and compared with the CQI over three years 2022, 2023, 2024.

Results: A total of 354 patient records were analysed (mean age 78.5 ± 6.3 years; 55.4% male). Compliance with all indicators exceeded 80%. Notably, 98% of patients had no emergency department visits, and 97% had no hospital admissions. Care consolidation was achieved for 83.3%, and 87% did not experience a fall within one year. Additionally, 95% avoided dopamine-blocking agents, and 91% received early physiotherapy review. The overall CQI was 57.3% for the duration of data collection. From 2022-2024, the CQI steadily improved from 55% in 2022 to 57% in 2023 to 60% in 2024. At the same time, mean cost per patient decreased from $3583 in 2022 to $2970 in 2023 to $2756 in 2024.
Conclusion: Our model of care demonstrates improving quality outcomes delivered at lower per-patient cost, consistent with enhanced value of care over time. Multidisciplinary care plays an important role in appropriate and value-based Parkinson’s Care delivering meaningful outcomes aimed at reducing healthcare utilization, promoting care consolidation and appropriate care for older adults with Parkinson’s disease.
