Corticobasal Degeneration With Alien Limb Phenomenon and Severe Cognitive Decline: A Levodopa-Resistant Clinical Case

Objective: To describe a clinical case of corticobasal degeneration presenting with alien limb phenomenon, severe cognitive decline, and resistance to high-dose levodopa therapy.

Background: Corticobasal degeneration (CBD) is a rare, progressive neurodegenerative tauopathy characterized by asymmetric parkinsonism, cortical dysfunction, and cognitive impairment. The alien limb phenomenon represents a distinctive but uncommon manifestation, reflecting cortical motor disintegration and posing significant diagnostic and management challenges.

Method: A 72-year-old patient with a five-year history of progressive neurological deterioration was admitted with marked cognitive impairment, including deficits in memory, attention, and executive function. Involuntary purposeless movements of the left upper limb were observed. The patient persistently rubbed the forehead with the left hand, consistent with alien limb phenomenon, resulting in a chronic friction-induced skin ulcer.

Results: Neurological examination demonstrated advanced cognitive decline, asymmetric motor involvement, and involuntary left-hand activity perceived as non-volitional. Despite treatment with levodopa at a total daily dose of up to 1500 mg, no clinical improvement was observed. The constellation of alien limb phenomenon, asymmetric parkinsonism, prominent cortical cognitive deficits, and levodopa resistance supported the diagnosis of corticobasal degeneration.

Conclusion: This case highlights the diagnostic value of alien limb phenomenon combined with levodopa-resistant parkinsonism and progressive cognitive decline in corticobasal degeneration. Early recognition facilitates appropriate supportive management, prevention of secondary complications such as self-inflicted skin injury, and improved patient and caregiver counseling.
