Objective: The aim of the present study was to know if a global assessment of Parkinson’s disease (PD) using a new proposed index correlated with changes in quality of life (QoL) and autonomy for activities of daily living (AADL).
Background: PD is a complex neurodegenerative disorder characterized by a combination of motor symptoms, non-motor symptoms, and cognitive impairment. All of these progress over time and impact QoL and AADL, so the patient assessment must be comprehensive.
Method: Using data of PD patients from specific scales (UPDRS [Unified Parkinson´s Disease Rating Scale]; NMSS [Non-Motor Symptoms Scale]; BDI-II [Beck Depression Inventory II]; FOGQ [Freezing of Gait Questionnaire]; PD-CRS [Parkinson´s Disease Cognitive Rating Scale]) from the COPPADIS cohort (references 1 and 2), firstly, we applied a score at baseline (V0) and at 5-year follow-up (V5) defined as Global Parkinson´s Symptoms Score (GPSS = [UPDRS-III-OFF + UPDRS-IV + NMSS + BDI-II + FOGQ / PD-CRS] x 100). Secondly, we applied an index to evaluate disease progression at V5, defined as the Global Parkinson´s Symptoms Index (GPSI = [GPSS at V5 – GPSS at V0 / GPSS at V5] x 100). QoL and AADL were assessed with the PDQ-39SI, PQ-10 and EUROHIS-QOL8, and the Schwab and England scale (ADSL), respectively. Correlation coefficients and non-parametric statistical tests were applied.
Results: A strong correlation was observed between the PDQ-39SI and the GPSS at V0 (r=0.720; N=606; p<0.0001) and at V5 (r=0.780; N=345; p<0.0001). Moderate significance were observed for the rest of the correlations at V0 and at V5 (Table 1). The GPSS increased significantly from 80.1 ± 54.7 at V0 to 146.3 ± 134.5 at V5 (N=317; p<0.0001). At V5, 56 patients out of 317 patients (17.7%) had a negative GPSI (Figure 1), with no impairment in QoL (PDQ-39SI, PQ-10, EUROHIS-QoL) compared with the group with a positive GPSI and with a significantly lesser impairment in AADL (Figure 2). Moderate correlations were found between the GPSI and the change from V0 to V5 in the PDQ-39SI, EUROHIS-QOL8, and the ADSL (Table 1).
Conclusion: GPSS and GPSI correlated with QoL and AADL, suggesting that they could be useful to monitor the progression of PD.
