Chinese Expert Consensus and Recommendations on Sensing-guided Deep Brain Stimulation for Parkinson’s Disease: A Modified Delphi Study

Objective: To propose some well-accepted recommendations on sensing-guided deep brain stimulation for Parkinson’s Disease.

Background: Deep brain stimulation (DBS) has been widely accepted as a well-established neurosurgical intervention for advanced Parkinson’s disease (PD) with motor complications. The recent emergence of DBS devices equipped with local field potential (LFP) sensing capability revolutionizes programming and propels DBS therapy into the era of personalized, precision medicine.

Method: These recommendations were developed by leading young specialists in PD neuromodulation practice across China, based on robust evidence-based medical principles and extensive clinical experience with sensing-guided DBS. They were then rigorously refined through multiple rounds of discussion and consultation using the modified Delphi method.

Results: They comprehensively address the fundamental principles and functions of sensing technology, essential preoperative communication regarding device selection, critical aspects of perioperative and postoperative LFP monitoring, initial and long-term programming strategies leveraging LFP, data utilization and safety protocols, as well as current technical limitations and future prospects.

Conclusion: These recommendations aim to empower neurologists and neurosurgeons to standardize and optimize the integration of sensing-guided DBS into exemplary clinical practice.
