Impact of Continuous Subcutaneous Foslevodopa/Foscarbidopa (FLC/FCD) on Non-Motor Symptom Burden in Advanced Parkinson’s Disease: Domain-Level Analysis

Objective: To comprehensively assess changes in non-motor symptom domains and MDS-UPDRS non-motor experiences of daily living following continuous subcutaneous FLC/FCD initiation.

Background: Non-motor symptoms (NMS) significantly impair quality of life in advanced Parkinson’s disease (PD), yet response to device-aided therapies remains incompletely characterized.

Method: Twenty-eight advanced PD patients were under went inpatient FLC/FCD initiation. Non-motor outcomes were evaluated using Non-Motor Symptoms Scale (NMSS; n = 15 with paired data) and MDS-UPDRS Parts I+II (n = 11 with paired data) at baseline and 6-month follow-up. Domain-level changes were analyzed.

Results: Total NMS burden improved significantly (−30.4 ± 19.4 points, 49.2% reduction, p = 0.0007; continuers: −33.3 ± 19.2, 57.7% reduction, p = 0.0005). MDS-UPDRS non-motor sub scores (Parts I+II) decreased markedly (−22.7 ± 9.3 points, 50.1% reduction, p = 0.001). Domain-level analysis revealed significant improvements across multiple NMS domains: cognition (−4.1 points, p = 0.005), anxiety (−3.4, p = 0.0075), pain (−3.0, p = 0.005), depression (−2.9, p = 0.008), urinary symptoms (−2.7, p = 0.008), apathy (−2.7, p = 0.018), physical fatigue (−2.0, p = 0.004), and impulse control disorders (−1.7, p = 0.007). The largest MDS-UPDRS domain improvements occurred in pain (−2.3, p = 0.002), sleep (−1.8, p = 0.016), daytime sleepiness (−1.5, p = 0.008), freezing (−1.5, p = 0.008), turning in bed (−1.3, p = 0.004), and speech (−1.2, p = 0.002).

Conclusion: Continuous subcutaneous FLD/FCD yielded robust, broad-spectrum improvements in non-motor symptom burden, particularly in pain, cognition, anxiety, sleep disturbances, and activities of daily living. These findings support the comprehensive therapeutic benefit of continuous levodopa delivery beyond motor symptom control.
