Objective: The aim of this study is to find a correlation between motor and non-motor symptoms fluctuation in parkinson’s disease.
Background: Despite increasing awareness of non-motor symptoms in Parkinson’s disease, non-motor fluctuations remain under diagnosed and insufficiently characterized in routine clinical practice. Moreover, the relationship between non-motor and motor symptom fluctuations remains incompletely understood.
Method: cross sectional observational study within Ain Shams university Movement disorders clinic .After a formal written consent all patients were subjected to Non-Motor Fluctuation Assessment (NoMoFA) questionnaire, Wearing Off Questionnaire (WOQ-19), Unified Dyskinesia Rating Scale (UDysRS), Modified Hoehn–Yahr score and MDS-UPDRS Part III.
Results: Among 58 patients, the total NoMoFA score showed a mean of 32.50 ± 17.69, and a range from 3 to 72, indicating moderate non-motor fluctuation severity among the studied patients. With total number of symptoms range from 1 to 26. The total on score were minimal, with a mean of 1.10 ± 2.59, whereas the total off score were higher, showing a mean of 9.47 ± 11.98 ,so The total non-motor fluctuations (NMF) score had a mean of 10.57 ± 13.16, while the total static non-motor symptoms (NMS) score was higher, with a mean of 23.05. Regarding levodopa equivalent dose are significantly associated with the severity of NMF. Regarding motor symptoms assessment, these results suggest that greater motor severity, particularly in the off state, is associated with higher NMS (static) score. For motor fluctuation we used UDysRS, These findings emphasize that dyskinesia contribute to the severity of total NMF score. Also, wearing off scale shows highly significant correlation between wearing off phenomenon of motor and non-motor symptoms and Total NoMoFA and NMF (dynamic) scores.
Conclusion: There’s a strong relationship between severity of non-motor and motor symptoms fluctuation in Parkinson’s disease. Also, the more sever motor deficit, the higher stationary non-motor symptoms.
