Association Between Liver Function and Motor Symptoms in Parkinson’s Disease: A Cross-cohort Analysis of Chinese and PPMI patients

Background: Emerging evidence implicates liver dysfunction in Parkinson’s disease (PD) pathogenesis, yet population-specific variations in this relationship remain poorly characterized.

Method: We retrospectively analyzed data from 514 PD patients (279 males, 235 females) at West China Hospital of Sichuan University (2008-2025). Baseline liver function parameters were compared with those from PPMI. In the PPMI cohort, we further performed correlation analyses to examine the relationship between liver function and motor symptom severity.

Results: The Chinese and PPMI cohorts exhibited comparable demographic and clinical baseline characteristics. In the PPMI cohort, after adjusting for age and sex, ALT showed a negative correlation with UPDRS Part III scores (ρ= -0.093, 95% CI [-0.149, -0.036], P = 0.001). No significant associations were observed between AST or total bilirubin and UPDRS Part III scores.

Conclusion: ALT is inversely associated with motor severity in PD, suggesting a potential protective or metabolic modulatory role. These preliminary findings underscore the need for prospective validation, particularly in Chinese populations, to elucidate ethnic variations in liver-brain axis interactions.
