Objective:
Examine whether digital monitoring within Advanced Primary Care Management (APCM) can detect between-visit change, prompt care-plan updates, and streamline longitudinal management for medically complex patients with Parkinson disease.
Background:
Outpatient management of motor and nonmotor symptoms of Parkinson disease is complicated by risk of functional decline and acute exacerbation, with poor outcomes and high cost of care, yet episodic visits capture only a fraction of clinically relevant interval change. APCM offers one U.S. framework for structured care of medically complex patients, and analogous coordinated-care models exist internationally. Implementation depends on efficient interval data capture, rapid identification of patients needing action, and timely translation of new information into care-plan changes. Digital monitoring may help operationalize that workflow.
Method: A single community neurology clinic will enroll up to 200 patients and combine APCM with a digital Parkinson disease platform already used by thousands for self-management. The platform integrates objective and patient-reported data, including symptom reports, medication logs, function, safety events, and targeted screeners deployed programmatically. Monthly review will map interval data to care-plan domains within the electronic health record, support low-touch management for stable patients, and flag alerts that trigger stepped outreach and clinician review proactively for declining patients. Feasibility measures include data capture, care-plan updates identified, documentation completion, review time, escalation frequency, outreach burden, acceptability, and operational cost neutrality.
Results:
Program launch is underway with planned enrollment of up to 200 patients. Program design defines the target population, monthly review cadence, escalation pathway, and evaluation framework. The small-clinic setting will test feasibility of integrated remote monitoring within structured care management.
Conclusion:
Program implementation will test whether a scalable care-plan workflow augmented by digital monitoring can make Parkinson disease care more proactive in a small-clinic setting. Findings may inform APCM in the United States and other coordinated Parkinson care models requiring efficient between-visit surveillance and care-plan review.
