Non-Motor Outcomes of Unilateral Magnetic Resonance-Guided Focused Ultrasound Surgery in Parkinson’s Disease: A Meta-Analysis

Objective: This study aimed to assess the efficacy of the procedure specifically in relation to non-motor outcomes such as quality of life, disability scores, effect on ADLs, neuropsychological outcomes and cognition. Within this meta-analysis we analysed the outcomes over a period between one and twelve months post-intervention.

Background: Magnetic resonance-guided focused ultrasound (MRgFUS) has emerged as an alternative to current options for those with medication-refractory Tremor Dominant Parkinson’s disease (PD). There has been limited investigations into non-motor outcomes following MRgFUS in patients with PD.

Method: A systematic search of literature to identify studies reporting outcomes following a search of Web of Science and Cochrane library databases, in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Data analysis was then performed in R, utilising a random-effects models.

Results: Fifteen studies met the inclusion criteria with a total of 288 patients undergoing MRgFUS. Improvements were seen up to 12 months post-MRgFUS in the UPDRS Part I and II scores, CRST Part C scores, quality of life outcomes, LEDD and some neuropsychological outcomes; however, these improvements were mainly not statistically significant. No change was found in cognition outcomes, and some neuropsychological outcomes such as the Starkstein apathy scale. Insufficient data existed to assess sleep outcomes.

Conclusion: Our meta-analysis suggests potentially promising improvements in non-motor outcomes following MRgFUS in patients with Parkinson’s disease. However, the study highlights the need for future RCTs on this subject, which investigate non-motor outcomes.
