Objective: To describe a rare presentation of olfactory groove meningioma.
Background: Olfactory groove meningioma is a benign intracranial tumor arising from the dura of the anterior cranial fossa. It typically presents with symptoms related to frontal lobe dysfunction, including cognitive decline, personality changes, headaches, visual disturbances, or anosmia. Movement disorders and unexplained recurrent falls are uncommon presenting manifestations and may delay diagnosis.
Method: case report
Results: Case Presentation:
A 61-year-old woman presented with recurrent unexplained falls accompanied by a progressively worsening dystonic head tremor. She had no prior history of neurodegenerative disease or primary movement disorder. Neurological examination revealed a “no–no” head tremor with a null position and sensory trick, findings consistent with dystonic tremor. No focal motor weakness or sensory deficits were identified. Brain magnetic resonance imaging demonstrated a large extra-axial mass in the anterior cranial fossa, consistent with an olfactory groove meningioma, exerting significant mass effect on the bilateral frontal lobes.
Conclusion: This case highlights an unusual presentation of olfactory groove meningioma with recurrent falls and dystonic head tremors. Clinicians should consider structural intracranial lesions in the differential diagnosis of new-onset movement disorders and unexplained falls, particularly when symptoms are progressive or atypical. Early neuroimaging is essential for timely diagnosis and appropriate management.
