Progression and disability in Parkinson’s disease: what else can be done?

Objective: to identify key modifiable factors in the treatment of patients with Parkinson’s disease (PD) that can slow clinical progression and disability.ity.

Background: PD is an incurable disease. Currently, medical care for patients with PD can only be symptomatic. While medical care, in accordance with clinical guidelines and national standards, is aimed at maximizing the patient’s quality of life, various factors, such as the staffing level of hospital neurologists and their level of qualifications, can influence the condition of patients with PD.

Method: In a cohort retrospective study, medical records of 1,480 patients with PD from a health insurance organization database over a five-year period were analyzed to determine whether patient care complied with developed treatment standards.

Results: patients were divided into two groups: Group 1 – with worsening of the stage of PD according to Hoehn and Yahr (progression), Group 2 – with worsening of social and professional adaptation (appearance or worsening of the degree of disability). Defects in the provision of medical care were identified that affect accelerated clinical progression and deterioration of social and professional adaptation. By analyzing the impact of the identified deficiencies in providing medical care to patients with PD, we identified the main factors that may accelerate the progression of PD [Table 1] and worsen the professional and social adaptation of patients [Table 2].

Conclusion: Modifiable factors in providing medical care to patients with PD were identified, the consideration of which may lead to a decrease in the rate of clinical progression and worsening social adaptation.
