Objective: To evaluate the change in daily “Good On” time and motor fluctuations during conversion of patients from IPX066 to IPX203 as an interim analysis of an ongoing real-world study.
Background: IPX203, a novel extended-release carbidopa-levodopa formulation, is designed to enhance levodopa delivery, improve absorption in the proximal small intestine, and extend the duration of clinical benefit. ELEVATE-PD evaluates the efficacy and safety of IPX203 under real-world conditions.
Method: ELEVATE-PD is an open-label phase 4 study where PD patients are switched from prior levodopa-based therapies (IR CD/LD, IR + COMT inhibitors, or IPX066) to IPX203. Following a 5-week dose-optimization period and a 1-week stable dosing, patients are followed for 12 months. Eligible participants had an average of ≥ 2.5 hours of daily “Off” time at baseline. The primary endpoint is change in daily “Good On” time from baseline/Visit 1 to Day 42/Visit 4. Here we evaluate the real-world outcome in a subgroup of patients switching from IPX066 to IPX203.
Results: Twenty-four patients (mean age 65.5 ± 10.65 years) were included in the analysis. At baseline, mean daily “Good On” time was 9.55 ± 2.44 hours, and mean daily “Off” time was 5.93 ± 1.83 hours. Switching from IPX066 to IPX203 resulted in an increase in mean daily “Good On” time of 3.07 hours (n=22), accompanied by a 2.90-hour reduction in daily “Off” time. Notably, IPX203 increased the mean duration of continuous “Good On” intervals from 3.45 hours at baseline (n=24) to 6.58 hours at Visit 4 (n=22, 3.09 hours increase) and reduced daily motor fluctuations from 4.75 to 2.79 (40% reduction). Furthermore, switching to IPX203 was associated with a 10.3-point improvement in MDS-UPDRS total scores.
Conclusion: These data highlight IPX203’s ability to deliver more consistent and predictable symptom control when switching from IPX066, reinforcing its value as an effective treatment option for patients seeking improved management of motor fluctuations.
