Objective: To conduct a cognitive pretesting of a global, cross-cultural, multicentre (14 countries) Stepped Care clinical toolkit. The questionnaire is completed by the healthcare professional HDCP) and the patient/carer and has been developed from expert opinion, expert patient group feedback, and literature review.
Background: We have recently published the “Stepped Care” programme for a comprehensive assessment of Parkinson’s disease (PD) in the clinic so that key collateral “Vitals” of PD care are not missed in the consultations [1].
Method: The Stepped Care toolkit, which is aimed to be used as “clinical prompt” for all HCP, was explained by the investigators to PD doctors/nurses, as well as to patients and caregivers, across centres, within the framework of a service development initiative. After completion of the Stepped Care Toolkit questionnaire, an online cognitive pretesting of the questionnaire was also completed.
Results: In this ongoing study, thus far 40 HCPs and 20 patients/caregivers have provided feedback. Responses from both HCPs and patients/caregivers suggest the toolkit to be an effective tool to supplement clinical consultations. 48% of HCPs reported that the toolkit structure was too long, while 100% patients/caregivers felt the toolkit was of reasonable length. Survey of centres showed that, while the Western European centres were supported by residents and fellows, 25% of HCPs from developing countries had no support. 90% of HCPs felt the Stepped Care pathway was useful to improve their clinical practice, while 10% were unsure. Both HCPs and patients/caregivers agree the Stepped Care pathway has a major role in clinical practice and in empowering patients.
Conclusion: 95% of responses from global HCP’s suggest that the Stepped Care toolkit is acceptable for clinic use, while 100% of patients and caregivers endorse this. Once full pretesting data is available, a revised version of the toolkit will be available for global use.
