The ORCHESTRA Study Year 2: Practical Experiences With Foslevodopa/Foscarbidopa Treatment in Advanced Parkinson’s Disease in a Real-World Setting

Objective: To describe real-world learnings on the routine practical management of foslevodopa/foscarbidopa (LDp/CDp), a continuous 24-hour subcutaneous infusion therapy for advanced Parkinson’s disease (aPD), using multi-country retrospective, longitudinal data obtained from a patient support program (PSP).

Background: In aPD, many patients experience insufficient symptom relief and levodopa-related motor complications. For these patients, LDp/CDp is a nonsurgical option with flexible dosing of levodopa and carbidopa prodrugs. The efficacy and safety of LDp/CDp have been demonstrated in clinical trials, with patients experiencing statistically significant mean motor function improvement and decreases in Off-time compared to placebo. Most adverse events are non-serious and mild to moderate in severity. Complementing the clinical trial data, the PSP data offer an opportunity to capture practice-oriented insights in the largest real-world LDp/CDp dataset, which is important for improving patient experience.

Method: The Observational Retrospective CoHort study on Practical ExperienceS of Treatment with foslevodopa/foscaRbidopa in Advanced Parkinson’s Disease (ORCHESTRA) study utilizes longitudinal data recorded by AbbVie’s PSP nurses during routine interactions with patients with aPD receiving LDp/CDp. Analysis for year 2 includes assessment of patient sociodemographic data, infusion parameters, system management practices, treatment effectiveness, patient support, discontinuations, and safety outcomes within a large cohort. Additionally, selected outcomes such as dosing and optimization, system management practices, and reasons for discontinuation are compared to findings from the year 1 analysis to evaluate whether healthcare providers’ management strategies have evolved over time with increased experience, and the resulting impact on patient outcomes.

Results: More than 4,500 patients with aPD from 5 European countries are included in the dataset, which will be analyzed in 2026 and shared thereafter.

Conclusion: Ongoing analysis of longitudinal data from the PSP will provide a deeper understanding of real-world management of LDp/CDp in aPD over time and inform strategies for continuous improvement of the patient experience.
