Objective: To determine whether symptom profiles of levodopa-entacapone-carbidopa intestinal gel therapy can be effectively captured using continuous smartwatch-based monitoring compared to patient diaries and clinical assessments.
Background: Levodopa-entacapone-carbidopa intestinal gel therapy is established for reducing motor and nonmotor fluctuations in advanced Parkinson’s disease (APD), yet optimization of infusion flow rates remains largely empirical. Daily symptom variability is commonly missed during intermittent clinical assessments, which may preclude optimal dose titration. Consumer wearables offer a scalable approach to continuous monitoring that could improve clinical decision-making. If validated, such monitoring could enable more precise flow rate optimization and support development of closed-loop delivery systems.
Method: This prospective, single-centre pilot study will enrol five patients with APD on established levodopa-entacapone-carbidopa intestinal gel therapy at a neurology clinic in southern Sweden. Participants will wear smartwatches during waking hours running a CE-marked application for continuous monitoring of dyskinesia and tremor. Participants will attend monthly clinic visits over three months where smartwatch data are reviewed. The primary endpoint compares objective dyskinesia and tremor profiles from the smartwatch against diary times and clinical profile. Secondary endpoints assess whether digitally captured symptom profiles can inform flow rate and dose adjustments. Analysis will be primarily descriptive using graphical and concordance methods.
Results: Recruitment is anticipated in Q2 2026 with data collection through Q2-Q3 2026. Results will be reported upon study completion.
Conclusion: This study will provide preliminary evidence on the feasibility of continuous smartwatch-based monitoring for optimizing levodopa-entacapone-carbidopa intestinal gel therapy in APD.
