Anodal transcranial direct current stimulation could change the metabolomic profiles of cerebrospinal fluid in Parkinson’s disease

Objective: We tried to identify meaningful metabolite change after anodal tDCS in clinically diagnosed PD patients by adopting metabolomic analysis

Background: Transcranial direct current stimulation (tDCS) is a noninvasive technique that has been widely studied as an alternative treatment for Parkinson’s disease (PD). However, the precise mechanism of tDCS in PD remained to be clarified. Furthermore, there are few study investigating biological change followed by tDCS in PD patients.

Method: 14 PD patients were prospectively enrolled and underwent 5 session of anodal tDCS on M1 area in dominant hemisphere. Before and after 5 session of tDCS, we collected CSF from each subjects and compared metabolome change using liquid chromatography and mass spectrometry.

Results: The UPDRS-III, MoCA-K, and NMSS scores were significantly improved after anodal tDCS. (p < 0.01, p < 0.01, p < 0.01). In metabolomic analysis, We can confirm 364 metabolite using library search. Among them we selected 25 polar metabolites according to VIP score in each positive and negative ion mode. Especially, metabolites regarding fatty acid biosynthesis and purine/pyrimidine pathway, aromatic amino acid pathway, caffein pathway, and mitochondrial pathway were significantly changed after anodal tDCS.

Conclusion: We could confirm aodal tDCS could influence on change of metabolites in CSF, especially regarding to PD pathogenesis specific pathway. These findings indicates anodal tDCS has biological evidence for treatment of PD and further investigation is warranted.
