Mood and Apathy Outcomes after STN or GPi DBS for Parkinson’s Disease: An Interim Report from the ADROIT NAPA Sub-study

Objective: To summarize the effect of subthalamic nucleus (STN) or globus pallidus internus (GPi) DBS on mood and apathy outcomes in patients with Parkinson’s Disease (PD).

Background: While DBS improves motor outcomes in patients with PD, alterations in mood and apathy post DBS intervention have been reported. Possible precipitating mechanisms such as rapid medication reduction or current spread into neighboring structures or pathways have been postulated. ADROIT-NAPA sub-study evaluates mood and apathy in a large, real-world observational study.

Method: ADROIT is a large, international, prospective, post-market, observational study at up to 50 centers worldwide that collects long-term safety and effectiveness data on Abbott DBS systems in the real-world setting. The NAPA sub-study will evaluate non-motor and cognitive changes after DBS. Mood was evaluated with the Beck Depression Inventory (BDI) and apathy with the Starkstein Apathy Scale (SAS).

Results: As of October 2023, 340 PD patients have been enrolled with newly implanted DBS systems (313 STN; 27 GPi) at 38 sites in the US, Europe, and Asia-Pacific. DBS improved MDS-UPDRS III motor scores by 54% after STN DBS and 50% after GPi DBS at 1 year post implant. Average BDI scores were 11.3 ± 7.7 (n=275) at baseline and 10.8 ± 7.6 (n=158) at 1 year in the STN group and 13.5 ± 9.8 (n=15) at baseline and 12.1 ± 6.7 (n=8) at 1 year in the GPi group. Average SAS scores were 10.9 ± 7.6 (n=134) at baseline and 13.4 ± 9.5 (n=68) at 1 year in the STN group and 13.0 ± 7.8 (n=3) at baseline and 10.4 ± 4.0 (n=5) at 1 year in the GPi group.

Conclusion: The ADROIT global observational study provides a unique opportunity to evaluate a large number of patients after DBS, while the NAPA-sub-study addresses mood and apathy in patients with PD on a large scale. This interim report will summarize available 1-year paired data for non-motor and quality of life evaluations including BDI, SAS, and PDQ-39.
