Characteristics of Fatigue and Correlation with the Symptoms of Parkinson’s Disease

Objective: We aims to assess the frequency and severity of fatigue, to explore the clinical features and associated factors of fatigue, as well as to understand the influences on the motor and non-motor symptoms in patients with Parkinson’s disease (PD).

Background: PD is the second most common neurodegenerative disease mainly including motor symptoms and non-motor symptoms. Fatigue is considered one of the most common and poorly understood non-motor symptoms in PD patients, which may occur during all stages of PD even in the pre-motor period, and its prevalence often increases with the disease progression.

Method: A total of 128 patients with PD were enrolled and analyzed in current study. Fatigue severity scale (FSS) was used to evaluate the symptoms of fatigue, and subjects who scored > 4 were classified as fatigue. All patients were assessed the motor and non-motor symptoms by multiple scales. The results were comparison between the fatigue group and the non-fatigue group.

Results: Sixty (46.9%) patients were defined as fatigue. Fatigue was associated with UPDRS score (UPDRS-Ⅱ: p < 0.001; UPDRS-Ⅲ: p = 0.008, UPDRS-Ⅳ: p = 0.003) and Hoehn-Yahr stages (p = 0.002). On the other hand, fatigue was correlated with UPDRS-Ⅰ (p < 0.001), quality of life (p < 0.001), depression (p < 0.001), anxiety (p < 0.001), apathy (p < 0.001), sleep disorders (p < 0.001) and autonomic dysfunction (p = 0.027). Binary-Logistic regression analysis showed that the scores of UPDRS, anxiety and sleep disorders were independent risk factors for fatigue in patients with PD (UPDRS-Ⅱ: OR = 1.086, 95%CI = 1.035 – 1.140; HAMA: OR = 1.101. 95%CI = 1.046 – 1.159; PDSS: OR = 0.953, 95%CI = 0.932 – 0.974).

Conclusion: PD patients with fatigue have severer motor symptoms and motor complication, as well as more and severer non-motor symptoms. Anxiety and sleep disorders are significantly correlated with fatigue, and it therefore influences the quality of life.
