Objective: The objective of this narrative review was to evaluate the effectiveness of telemedicine and AI-based technologies used for the remote monitoring and management of Parkinson’s disease patients during the COVID-19 pandemic, with a focus on clinical outcomes, patient satisfaction, and technological performance
Background: The COVID-19 pandemic significantly disrupted neurological care for patients with Parkinson’s disease (PD), who require continuous monitoring. PD involves basal ganglia dysfunction associated with both motor and cognitive impairments. Reduced access to in-person care during the pandemic, combined with psychosocial stress, worsened cognitive and neuropsychiatric symptoms, accelerating the adoption of telemedicine and AI-driven digital health solutions to ensure continuity of care.
Method: A narrative review was conducted using studies published between 2020 and 2025, identified through searches in PubMed and Scopus. Eligible studies included clinical trials, observational studies, and real-world evaluations addressing telemedicine platforms, wearable devices, mobile applications, and AI-based systems for PD monitoring during and after the COVID-19 pandemic.
Results: In the investigated studies, telemedicine adoption surged by roughly 50–70% during the pandemic relative to pre-COVID eras. Virtual consultations effectively supplanted in-person visits in 60–80% of routine follow-ups, with patient satisfaction rates surpassing 80–90%. AI-driven solutions predominantly utilized machine learning algorithms on wearable sensor and smartphone data to quantify motor complaints, with classification accuracies between 75% and 95%. Deep learning methods, such as convolutional neural networks, were employed for video-based movement and voice analysis, while predictive models facilitated the early identification of symptom fluctuations and medication “off” times in 70–85% of observed patients. These techniques facilitated a decrease in hospital visits by around 30–50%.
Conclusion: Telemedicine combined with AI-based technologies proved to be an effective and reliable approach for Parkinson’s disease management during the COVID-19 pandemic. The integration of remote monitoring and AI-driven decision support enhanced continuity of care and clinical outcomes, supporting the long-term implementation of AI-enabled telemedicine beyond the pandemic context.
