Effectiveness of Lee Silverman voice treatment big on improving gait and quality of life in Parkinson’s disease patients after STN-DBS

Objective: To study effectiveness of Intensive amplitude specific exercise training of LSVT [Lee Silverman Voice Treatment] BIG on improving gait and quality of life in patients with advanced Parkinson’s disease after STN-DBS.

Background: In advanced Parkinson’s disease patients who has underwent STN-DBS, there is improvement in cardinal features of disease like bradykinesia, tremors, rigidity but long term studies has shown that there is deterioration in their gait and balance problems like increased freezing episodes of gait and increased risk of falls. There are limited studies regarding gait rehabilitation after DBS. Recent studies conducted on LSVT BIG showed improvement in gait in mild to moderate stage of PD patients. This study is to see effectiveness of LSVT BIG treatment in advanced Parkinson’s disease patients who has undergone STN-DBS.

Methods: Study Design – Experimental study. 20 subjects who have undergone STN-DBS at least 6 months prior to inclusion were taken and stabilized on constant current parameters and PD medications. The subjects in this group receive Intensive high amplitude exercise therapy sessions, 4 days a week for 4 weeks, every session lasting for 1 hour. Outcome Measures: The outcome measures used are Stride length, Time to walk 10 meters, Time up and go test, PDQ-39, UPDRS-3.

Results: 20 subjects were taken into study, 4 females and 16 males (F:M=20:80) with the mean age of 56.7500. All parameters are measured on 1st day and on 4th week after completing the treatment session. The data was collected and statistical analysis was done using Paired t-test. Stride length T-value is -15.771, Time to walk 10 meters T-value is 14.203, Time up and go test T-value is 11.85, PDQ-39 T-value is 11.456 and UPDRS-3 T-value is 20.29.There significant improvement in Stride length, Time to walk 10 meters, Time up and go test, PDQ-39, UPDRS-3 with their p-value <0.05.

Conclusions: This study illustrates that LSVT (Lee silver-man voice treatment) Big is effective in improving gait by this means improving quality of life in Parkinson’s disease patients after STN-DBS.
