Objective: We aimed to explore the possible differences between males and females as regarding motor, non-motor, depressive symptoms, cognitive functions and Transcranial Parenchymal Ultrasound (TCS) findings in a sample of Egyptian idiopathic Parkinson’s disease (PD) patients
Background: Males were found to be 2 times higher in incidence of Parkinson’s disease (PD). In animal models of PD, The dopaminergic neurons in the substantia nigra and its included dopamine were more liable to be injured at low oestrogen than at high oestrogen state.
Method: We recruited 109 patients fulfilled the criteria for diagnosis of idiopathic PD based on British Brain Bank criteria. We used Unified Parkinson’s disease Rating Scale (UPDRS), Montreal Cognitive Assessment (MOCA) , Non Motor symptom Scale (NMS), Scales for Outcomes in Parkinson’s Disease-Autonomic questionnaire (SCOPA-AUT ) and Hamilton Depression test (HAMD) to assess severity of motor ,cognitive, non-motor, autonomic symptoms and depressive symptoms respectively. TCS was used to assess echogenicity of Substantia Nigra (SN) and diameter of third ventricle.
Results: The recruited PD sample included 75 males (68.8%) and 34 Females (31.2%).
Male group was found to have significantly higher H&Y (2.76 ± 1.19 Vs 2.18± 1.11 P-value 0.017), UPDRS (81.09 ± 32.56 Vs 63.71 ± 30.89 P-value < 0.01), SCOPA (22.2 ± 9.56 Vs 17.97 ± 9.5 P-value 0.34) and HAMD (17.53 ± 9.05 Vs 13 ± 6.96 P-value 0.01) than Female group.
No significant difference between both groups regarding MOCA (21.33± 4.37 Vs 21.03 ± 4.88, P-value 0.747), total NMS (96.94± 35.23Vs 81.23±45.61P-value0.052).
Males had higher SN echogenicity (cm2) (Right SN 0.23±0.1 Vs 0.18±0.08 P-value 0.03 , Left SN 0.23 ± 0.1 Vs 0.18 ± 0.07 P-value 0.04) than Females. No significant difference between both groups regarding third ventricle diameter (4.26± 1.74 Vs 4.9±1.72 P-value 0.1) .
Conclusion: Idiopathic PD male patients were found to have worse motor, autonomic and depression scores than female PD patients.
