Stabilometry characteristics as objective Parkinson’s disease progression biomarkers

Objective: We aimed to find stabilometry characteristics, which may be used as disease progression and postural instability early development markers.

Background: Balance and gait disorders characterize progression of Parkinson’s disease (PD),predict the risk of falling,and contribute to reduced quality of life. Objective balance metrics may provide useful biomarkers for PD progression. The most commonly used tests for postural instability evaluation are stop-watch measures of gait speed or clinical balance rating scales.

Methods: 100 subjects with PD (Hoehn-Yahr stage 1-3) were assessed by the computerized static and dynamic posturography protocol (Diers,Germany).We assessed Hoehn-Yahr scale,total UPDRS,PIGD scale,disease duration.

Results: The statokinesiogram square size correlated with the PD duration (r=0,62,p<0,005), Hoehn-Yahr stage (r=0,71,p<0,05). Patients with PD showed an increase of statokinesiogram square,gravity center deviation in the frontal plane,basic oscillation spectrum frequency in the frontal and sagittal planes, which are more expressed in the advanced stages of the disease. We distinguished separate group of 30 subjects with PD in early-stage (PDES) (Hoehn-Yahr stage 1.0-2.0) without clinical postural instability and evaluated motion changes in the gravity center movement in order to find prognostically adverse balance indicators. PDES group was clustered in 3 subgroups according to«GC Movement», «GC movement maximum speed», «Oscillation surface» with K-average method. 1st subgroup included patients with the highest amplitude of GC movement, GC movement maximum speed and the biggest oscillation surface square. 2nd subgroup included patients with average values. 3rd group consisted of patients with relatively low values. In the 1st subgroup average PIGD score was 2,75, in the 2nd subgroup–3,16, in the 3rd subgroup–3,18. There were more patients at 2 stage by Hoehn-Yahr scale in the 3 subgroup than in others.

Conclusions: These data showed relationship between statokinesiogram square, PD duration and stage. GC Movement, GC movement maximum speed, Oscillation surface are candidates for early postural instability biomarkers. We plan to hold stabilometry in dynamics. In the future, the most useful balance and gait biomarkers for PD will be those sensitive and specific for early PD and disease underlying process related.
