Quantitative MRI measurement of regional cerebellar involvement in patients with progressive supranuclear palsy

Objective: We aimed to evaluate cerebellar involvement in PSP using MRI segmental volumetric analysis.

Background: Progressive supranuclear palsy (PSP) is characterized clinically as postural instability with early falls, supranuclear gaze palsy, symmetric bradykinesia, axial rigidity and dementia. Although early cerebellar symptom is one of exclusive criteria for PSP, some studies suggested that cerebellar involvement in PSP is evident in both clinically and pathologically. However, the topographical involvement of the cerebellum in PSP is not well recognized yet.

Methods: We enrolled retrospectively 50 patients with PSP which included 27 patients with PSP-Richardson’s syndrome (RS) and 23 patients with PSP-pure akinesia with gait freezing (PAGF), 40 patients with Parkinson’s disease (PD), and 34 healthy control subjects, who underwent structural MRI for an initial diagnostic work-up. MRI volumetry was performed using CIVET pipeline software. Data on both of whole and segmented cerebellar volumes were analyzed using an analysis of covariance.

Results: A general linear model showed that whole cerebellar volume in patients with PSP was significantly smaller compared with those with PD or healthy control after controlling age, gender, and intracranial volume (p = 0.028, post hoc p=0.017 and 0.031). In analysis of segmented cerebellar volume, patients with PSP exhibited decreased regional volume in the left crus I, lobule VIIIa and lobule VIIIb, which are known to play a role in eye movement and secondary motor representation (p = 0.022, 0.014, and 0.004). In subgroup analysis of PSP, whole cerebellar volume was smaller in patients with PSP-RS compared with those with PSP-PAGF. Additionally, patients with PSP-RS exhibited smaller segmental cerebellar volume in bilateral curs I compared with those with PSP-PAGF.

Conclusions: These data demonstrate that compared with healthy control or PD patients, cerebellar atrophy is evident in PSP patients and especially prominent in PSP-RS group.
