Objective: To explore the cross-sectional cognitive characteristics of the COPPADIS cohort and to identify associations between cognitive performance and lifestyle, neuropsychiatric symptoms, NM features and potentially treatable factors.
Background: COPPADIS (COhort of Patients with PArkinson’s DIsease in Spain) is an observational, 5-year follow-up,nationwide, multicenter study aimed to elucidate the course of Parkinson’s disease (PD) an to identify biomarkers to predict disease-related complications.
Method: From the cross-sectional database with 694 patients, we selected those with age,disease duration, total LD and DA treatment duration and equivalent daily dose, H&Y stage, UPDRS-III, NM symptoms scale(NMS), Beck Depression Inventory-II(BDI-II),QUIP-RS,PDSS, PDQ-39,MMSE,NPI, and PD-CRS data. We also collected data regarding RBD, falls, freezing, regular cognitive and physical exercise. Blood testing variables were also recorded. Patients with history of anemia or other neurologic disorders were excluded.
Results: A total sample of 533 patients was studied(mean age=62±8;disease duration=5±6;UPDRS-III(off)=22±11;H&Ystage=15%stage1;9%stage1.5;60%stage2,7.1%stage2.5;7.8%stage3,and 1.3 stage4). The PD-CRS classified 393 patients as cognitively preserved(PD-NC),114 as PD-MCI(21%),and 26 as PDD(4.8%).PD-NC were significantly younger than PD-MCI/PDD (p<0.05). All PD-CRS subtests significantly differentiated cognitive groups. The PD-NC group showed better scores in the total NMSS,BDI-II,PDSS,PDQ-39,and NPI dysphoria and apathy. No differences were found in these measures between PD-MCI and PDD. Clinically relevant apathy was found in 21% of PD-NC,35% of PD-MCI and 43% of PDD. Linear regression analysis showed a significant association between cognitive performance and severity of depressive symptoms, homocysteine, B12,and iron levels. Significant differences were found between cognitive groups in the frequency of falls and freezing. Practice of regular exercise and cognitive exercise were both associated with better PD-CRS subcortical score.
Conclusion: In the COPPADIS cohort the prevalence of PD-MCI in early PD patients is similar than previously reported. Early cognitive changes have a significant impact on QoL and are influenced by depressive symptoms,frequency of falls and freezing,regular exercise and cognitive stimulation,and levels of homocysteine, B12, and iron.
