A Risk of Thromboembolic Complications In Patients With Parkinson’s Disease

Objective: A small number of reports refer to studies assessing the risk of thromboembolic complications in patients with PD in the form of a stroke, myocardial infarction, or deep vein thrombosis. An analysis of the literature shows conflicting views regarding the occurrence of blood coagulation in the course of PD.

Background: The condition of the fibrinolytic system, which is a natural compensatory mechanism important to maintaining the balance of the hemostasis process, determines the appearance of a clinically expressed thrombotic process.

Method: The study group consisted of 40 patients diagnosed with PD treated in University Hospital No. 2 and Neuromed Medical Center, Bydgoszcz. The control group consisted of 30 adult volunteers aged 55-80 years.

Data on the advancement stage of the disease (UPDRS, Hoehn Yahr (HY)scale) and basic biometric measurements were obtained.

The following parameters were determined using ELISA test: concentration of tissue and urokinase plasminogen activator antigen(t-PA: Ag, u-PA: Ag), u-PA activity, concentration of plasminogen activator inhibitor antigen type 1 and 2 (PAI-1: Ag,PAI-2: Ag), concentration of t-PA / PAI-1 complexes and D-dimer (D2D).

Results: The study showed a significantly higher concentration of t-PA: Ag. The increase concentration of PAI-1: Ag and D-dimer in the advanced stage patients group and PAI-2:Ag was lower, compared to the control group. An analysis of the correlation between the tested fibrinolysis parameters and the scores of the scales used to assess the severity of PD showed a decrease in tPA:Ag concentration with disease progression, as indicated by the HY scale and part IV of the UPDRS scale, and an increase in PAI-1:Ag concentration, which results from the score obtained using the UPDRS scale as well as an increase in D-dimer concentration with the progression of the disease, as indicated by the Hoehn Yahr and UPDRS scales.

Conclusion: Increased concentration of D-dimers in the blood of patients with PD indicates a secondary activation of the fibrinolysis process to an earlier activation of clotting. Increased concentration of PAI-1 in the blood of patients with advanced PD inhibits the fibrinolytic system and may be related to the intensification of alpha-synuclein formation.
