An elderly female with subacute cerebellar ataxia: A case report

Objective: To report the case of a 62year old woman who presented with rapidly progressive ataxia.

Background: Paraneoplastic cerebellar syndrome should be suspected in elderly patient presenting with subacute cerebellar ataxia. Usually, it is associated with various types of cancer(gynecological and lung cancers).

Method: a case report

Results: 62 year old female ,withou personal or familial medical history, presented with subacute onset pancerebellar syndrome since 2.5 months in form of cerebellar speech with gait and appendicular ataxia. CEMRI brain showed diffuse cerebral atrophy,csf study showed 30 cells, with raised protein(124mg/dl) and normal sugar. serum TSH, anti tpo, ANA panel, autoimmune encephalitis panel was negative. Serum paraneoplastic was strongly positive for anti YO. source of malignancy was not found despite FDG-PET and other investigations. Thus diagnosis of probable paraneoplastic cerebellar degeneration ( PNS – care score-7) was made.so ,we observed that PCD emerged even before any malignancy diagnosis.

Conclusion: Paraneoplastic cerebellar syndrome should be suspected in elderly pt presenting with subacute cerebellar ataxia after excluding other causes even in the absence of tumor at presentation.

When initial tumor screening is negative ,it should be repeated every 4-6 months for 2 years in patients with high risk phenotypes along with high risk antibodies.
