Characteristics of Pain in Parkinson’s Disease with Arabic translation and validation of King’s Parkinson’s Disease Pain Scale

Objective: To translate and validate the King’s Parkinson’s Disease Pain Scale (KPPS) into Arabic and to correlate pain among PD patients with motor and non-motor symptoms (NMS).

Background: Parkinson’s disease (PD) affects 3-5% of the population. It consists of motor and non-motor symptoms (NMS). Pain is one of the most common NMS in PD affecting up to 85%.

Method: 45 PD patients were assessed by KPPS after its Arabic translation and validation and compared to 40 healthy controls. Patients were also assessed by MDS-UPDRS, Non-Motor Symptoms Scale, Parkinson’s Disease Questionnaire for quality of life (PDQ-39), Beck Depression Inventory, and the Non-Motor Fluctuation Assessment (NoMoFA) Questionnaire.

Results: All PD patients had at least 1 type of pain and 77.77% had ≥3 types. The mean patients’ total KPPS score was 39.73±25.92 (p<0.001). there was no intra rater statistical difference indicating fair reliability and validity. Fluctuation-related pain reached 75.56% followed by musculoskeletal pain 73.33%. Factors associated with higher KPSS scores were duration (p=0.022), depression (p=0.028), PDQ-39 (p=0.005), severity of disease, impaired mobility, and fluctuation of NMSs (p<0.001). Pain subdomains positively correlated with fluctuation were chronic pain, nocturnal pain, fluctuation related pain, orofacial pain and pain related to discoloration and oedema. PIGD subtype had higher musculoskeletal pain compared to tremors subtype. Early onset patients had higher fluctuation related pain and nocturnal pain.

Conclusion: The current study presents a valid and reliable Arabic version of KPPS. PD patients suffer higher degree and prevalence of pain that are associated with disease duration, severity, depression, and impaired mobility, with negative impact on QoL.
