Objective:
To compare the frequency, the clinical profile of parkinsonism and its etiologies in 2008 and 10 years later in our department.

Background: Number of patients with parkinsonism increases over time, its main etiology is Parkinson’s disease (PD). But in Madagascar, there are few studies about parkinsonism, this may be due to the low number of neurologists who practice there. In recent years, number of neurologist has increased and some have become interested in movement disorders. So, in this study, we wanted to show the difference between one year where neurologists were in short supply and the year when certain neurologists were interested in movement disorders, and we also wanted to see how the profile of patients with parkinsonism is evolving over time.

Method:
This is a cross sectional study, from patients records of 2008 and 2018 in the Neurology department of Befelatanana University Teaching Hospital, Antananarivo. Demographic and clinical data of patients with parkinsonism were collected. Etiologies of parkinsonism were classified in PD, parkinsonism plus (degenerative etiologies other than PD) and secondary parkinsonism (non degenerative etiologies). Hospital frequency of parkinsonism (relative to other neurological disorders), its clinical profile and etiologies in 2008 and 2018 (10 years later) were compared.

Results:
Compared to 2008, parkinsonism in 2018 had higher hospital frequency 5% (46/921) versus 0.27% (3/1073), its number increased about 15 times after 10 years; a higher mean age (SD) at diagnosis 62 (9,05) years old versus 57 (12,73) years old (p=0.452) but not significant ; a higher mean age (SD) at onset 58,74 (9,78) years old versus 54 (8,49) years old (p=0.003); a slightly lower diagnosis delay 3,36 years versus 3,5 years (p=0,959) but not significant ; in both years there were male preponderance at 100% (3/3) and 56,52% (26/46) of cases. the most represented clinical features and etiologies in 2018, were tremors in 22/46 (47.42%) and PD in 40/46 (87%) versus rigidity in 2/3 (66.66%) and parkinsonism plus in 2/3 (66.66%), in 2008.

Conclusion:
We found that in our country, the number of parkinsonism increased, and its profile tended to follow the other countries’ over time. The increase of neurologists’ number interested in that field contributed to that.
