Cortical impairment as marker of dementia risk in probable REM sleep behavior disorder in Parkinson’s disease

Objective: To assess utility of Parkinson’s disease-cognitive rating scale (PD-CRS) to detect Mild cognitive impairment (MCI) and dementia risk in a Parkinson’s disease (PD) subpopulation.

Background: REM sleep behavior disorder (RBD) is a parasomnia associated with MCI on neuropsychological testing in PD. Follow-up studies have demonstrated higher risk of dementia in patients showing MCI at baseline. MCI is associated to impairment in subcortical areas while progression to dementia involve impairment of cortical areas also. PD-CRS provide neuropsychological scores to assess cortical and subcortical functioning for detecting MCI and risk of dementia.

Methods: Twenty five consecutive PD patients(mean age 61.04, SD=7.86, 14 women, Hoehn & Yahr stage 1-2) were screened for probable RBD (p-RBD), using the RBD Screening Questionnaire and PD-CRS to assess cortical and subcortical cognitive impairment.

Results: p-RBD patients (n=9, 36%, cut-off ≥6) showed PD-CRS impairment (after education adjustment) in total score [(95% CI) 68.8-83.36, p=0.028), cortical score [(95%CI) 22.46-25.87, p=0.036] naming sub test [(95% CI) 14.18-16.96, p=0.028), and action fluency [(95% CI) 7.48-12.20, p=0.028).

Conclusions: PD-CRS is useful to detect MCI in p-RBD EP patients regarding total score. Cortical impairment account for dementia risk in p-RBD EP in early stages of the disease.
