Differences in cerebral perfusion between Major depression disorder and depression with Parkinson’s disease : Brain HMPAO SPECT study using SPM analysis

Objective:  We performed SPECT in Major depressive (MD) disorder and PD patients with (PDMD) and without depression (PDNMD). The aim of this work was to investigate regional cerebral blood flow (Rcbf) in patients with PDMD and PDNMD, and patients with MD, using 99mTc hexamethyl-propyleneamine-oxime single-photon emission computed tomography (HMPAO-SPECT). 

Background: Diagnosis of depression in PD is complicated by overlapping symptoms of the two disorders.

Methods: 103 patients were studied, 38 PDMD, 46 PDNMD and 19 MD patients. A diagnosis of idiopathic PD was made according to the UK Parkinson’s Disease Society Brain Bank Clinical Diagnosis Criteria. At inclusion all PDMD and MD patients were evaluated according to the structural clinical interview for a DSM-IV diagnosis of ‘‘major depressive disorder’’. All participants underwent brain HMPAO-SPECT imaging. The voxel wise whole-brain analysis were conducted to compare rCBF between the PDNMD, PDNMD and MD groups. of the two disorders. Results: The ratio of female in PDMD and MD group was significantly higher than PDNMD group. In addition, Geriatric Depression Scale (GDS-30) and neuropsychiatric inventory (NPI) scores of PDMD group was significantly higher than the PDNMD group. PDMD group showed more significant hypoperfusion in the subcallosal cortex compared to PDNMD group. The MD group showed significant hypopefusion in brainstem and partial frontal region compared to PDMD group. NMD, PDNMD and MD groups. of the two disorders. Conclusions: We suggested that dysfunction of inferomedial frontal cortex might be involved in the pathogenesis of depression in PD.
