Objective: We aim is to assess the efficacy of Lee Silverman Voice Treatment-BIG therapy (LSVT-BIG) compared to other physiotherapy in patients with Parkinson’s Disease (PwP).
Background: PD is a chronic neurodegenerative disease characterized by motor and non-motor symptoms. LSVT-BIG is a new approach that aims to increase movement amplitudes and improve motor symptoms in PwP.
Method: We conducted a comprehensive search across four electronic databases and included RCTs that compare LSVT with other physiotherapy in PwP. A uniform Google sheet was used for data extraction and quality assessments.
Results: We included eight RCTs with a total of 232 patients. LSVT-BIG could improve motor symptoms assessed by UPDRS score; however, this improvement was statistically insignificant compared to the control group (MD = -2.54, 95% CI [-5.22 – 0.14], P-value = 0.06). In subgroup analysis according to follow-up time, it still insignificant at 1 and 4 months. Patients in the LSVT group had significantly better 10-minute walk test improvement (MD = -0.54, 95% CI [-1.04 – -0.04], P-value = 0.04) compared to the control group, but there was no significant difference between the two groups as regards TUG, (MD = -0.40, 95% CI [-1.49 – 0.68], P-value = 0.47), PDQ-39 (MD = 0.68, 95% CI [-2.01 – 3.37], P-value = 0.62), and BDI (MD = -2.04, 95% CI [-4.50 – 0.43], P-value = 0.11).
Conclusion: LSVT-BIG had a beneficial effect in improving motor function and quality of life in PwP, but we could not prove its superiority to other physiotherapy. More RCTs with larger sample sizes are still needed.
