Objective: To describe manifestations of Parkinson’s Disease (PD) in different ethnicities in Sumatera, Indonesia.
Background: Prevalence of PD is varied around the world. Some studies reported that PD incidence in Asia was rather lower than in Europe and America. Other studies also reported that PD incidence was higher in certain ethnicities. Some reports suggested that there were probabilities of geographic and ethnic differences in clinical manifestations, epidemiology and mortality of PD. Prevalence of excessive daytime sleepiness (EDS) was reported less in Asian countries studies than in North America and Europe studies. There was also evidence that Black PD patients tend to have cognitive decline and dementia than other ethnic groups. However, it is still unclear what the nature and cause of these differences are. Indonesia is a multi-ethnic country. It is unknown if each ethnicity in Indonesia is also having different manifestations of PD. Only a few studies which reported clinical manifestations of PD in different ethnicities.
Method: We performed a cross-sectional descriptive study. Primary data were collected by interviewing the subjects and secondary data were collected from medical records of PD patients who visited Neurology Clinic in 3 hospitals in Palembang, South Sumatera. There were 73 PD patients participated in this study and fulfilled the inclusion criteria. The subjects had been categorized into Sumatera and non-sumatera ethnicity. We collected data on sociodemography, MMSE score, duration of disease, staging of PD, wearing off phenomenon, non-motor symptoms, family history of PD, protective and risk factors of PD.
Results: In 73 subjects, there were 48 subjects of Sumatera and 25 subjects of non-sumatera ethnicity. Most subjects did not show significant differences in clinical manifestations based on their ethnicity [table 1]. However, most subjects of sumatera ethnicity did not suffer from apathy and more likely suffered from EDS than non-sumatera ethnicity [table 2] and [table 3]. There were no significant differences in protective and risk factors of PD between sumatera and non-sumatera people.
Conclusion: There were different clinical manifestations of PD based on ethnic variations. Sumatera people mostly did not show any apathy symptoms, but rather frequently suffered from sleep disturbances of EDS than non-sumatera people.
