Movement disorders (MD) after stroke in childhood

Objective: The aim of this study is to characterize child population with MD as manifestation of both ischemic and hemorrhagic stroke.

Background: Many different types of movement disorders (MD) have been associated with ischemic or hemorrhagic stroke. In adults, MD are observed in 1-4% of patients with stroke, but the prevalence and characteristics of these manifestations in children have not been widely described

Method: Analysis of clinical records and review of videos of patients with MD during or after ischemic or hemorrhagic stroke from infancy to adolescence (birth to 18 years old) between 1990 and 2023, from the Pediatric Neurology department of San Borja Arriaran Hospital, Santiago Chile

Results: 70 pediatric patients with ischemic or hemorrhagic stroke were studied. 7 (10%) of them (6 men; 86%), mean age 3.2±1.5y, manifested MD as a clinical feature. All the cases studied were ischemic strokes. In 100% of the cases, the affected vascular territory was the middle cerebral artery (6 deep and 1 complete). The most frequent MD was chorea (n=4, 57%) and followed by dystonia (n=3, 43%). 3 patients (43%) presented MD as the initial clinical manifestation, and in 57% MD was observed as a late presentation. A good response to symptomatic pharmacological treatment was reported in 30% of cases.

Conclusion: In our series, 10% of pediatric patient with stroke manifested MD. Chorea was the most frequent symptom, followed by dystonia. Middle cerebral artery territory was affected in all patients. Most stroke-related MD had a late presentation. Only one third of the patient reported a response to symptomatic pharmacological treatment.
