Physiotherapy can reduce the decline in functioning according to ICF framework in patients in early stage of Parkinson’s disease evolution

Objective: To investigate the effects of physiotherapy treatment on functioning according to International Classification of Functioning, Disability and Health (ICF) framework in patients in early stage of Parkinson’s disease (PD) evolution.

Background: Several scientific evidences have showed the positive effects of Physiotherapy to reduce the motor deficiencies associated the symptoms of PD, mainly for patients in intermediate stages of PD evolution. However, the effects of PT to reduce the decline in functioning in early stages of PD evolution remain unclear.

Methods: Forty patients with PD (21 men and 19 women, age 45-78 years; median disease duration, 5 years) at stage 1 H&Y scale were divided in the Physiotherapy Group (PG) and Control Group (CG). The PG was constituted for patients who performed physiotherapy treatment, 1 session per week, at least for six months. The CG was constituted for patients who had not never performed any physiotherapy treatment for PD. Both groups were evaluated following the Parkinson EDGE Task Force Recommendations based on ICF framework: UPDRS – part 3 and Montreal Cognitive Assessment (MoCA) were used to assess Body Structure and Function; Six minute walk (6MWT), 10 meter walk (10MWT), Mini BESTest, UPDRS – part 2, Functional Gait Assessment (FGA), Five Times Sit To Stand (FTSTS), 9 hole peg test (9HPT) were used to assess the Activity level; and Parkinson’s disease Quality of Life (PDQ 39) was used to assess the Participation level.

Results: One-way ANOVA showed that PG patients achieved the significant higher performance than CG patients in the UPDRS – part 2 and 3; Mini BESTest; FTSTS and PDQ-39.

Conclusions: Physiotherapy treatment in early stages of PD evolution may prevent the decrease in the level of functioning in the Body Structure and Function, Activity and Participation domains according to ICF framework. This evidence supports the importance of indication of physiotherapy treatment for patients in early stages of PD evolution.
