Objective: The aim of the present study was to analyze the change in burden and strain after a 2-year follow-up in a cohort of caregivers of patients with Parkinson´s disease (PD)and also to identify predictors of these changes.

Background: Caregiver burden in PD has been studied in many cross-sectional studies but poorly in longitudinal ones.

Method: PD patients and their caregivers who were recruited from January/2016 to November/2017 from 35 centers of Spain from the COPPADIS cohort were included in the study. They were evaluatedagain at 2-year follow-up. Caregivers completed the Zarit Caregiver Burden Inventory (ZCBI) and Caregiver Strain Index (CSI) at 
baseline (V0) and at 2-year follow-up (V2). General linear model repeated measure and lineal regression models were applied.

Results: One hundred and ninety-two PD patients (63.96 ± 8.74 years old; 63% males) and their principal caregiver were included. Mean age of the caregiver was 58.82 ± 11.71 years old and 69.3% of them were females.After the 2-year follow-up, the mean ZCBI score significantly increased by 31.4% (from 13.2 ± 11.31 to 17.35 ± 13.44; <0.0001; Cohen´s effect size = 0.51) whereas the mean CSI score by 26.5% (from 1.89 ± 2.17 to 2.39 ± 2.05; p<0.0001; Cohen´s effect size = 0.34). The percentage of caregivers with little or no burden (ZCBI 0-20) decreased from 77.1% at V0 to 67.7% at V2 (p<0.0001). At V2, 8.3% of caregivers had high level of stress (CSI 7-13)compared to 4.7% at V0 (p=0.090). Caregiver´s mood impairment from V0 to V2 was the strongest factor associated with an increase from V0 to V2 on the total score of the ZCBI (β=0.416; p<0.0001) (Table 1) and CSI (β=0.277; p=0.001) (Table 2).

Conclusion: Burden and strain were impaired in caregivers of PD patients after a 2-year follow-up. Mood changes in the caregiver is a key aspectrelated to caregiver burden increase.
