Prospective observational study on the effects of physical exercise and lifestyle modifications in patients with Parkinson’s disease

Objective: This study aimed to assess the effectiveness of physical activity, which includes all movements that increase energy expenditure, in conjunction with exercise, characterized as organized and systematic physical activity, and lifestyle modification counselling based on a comprehensive lifestyle intervention model, concerning adherence and tremor management in individuals with Parkinson’s disease.

Background: Parkinson’s disease (PD) is a progressive neurodegenerative disorder characterized by motor and non-motor symptoms that significantly impact quality of life. Emerging evidence suggests that lifestyle modifications, including physical exercise, dietary interventions, and holistic lifestyle changes, play a crucial role in symptom management.

Method: This prospective study was conducted at the Movement Disorder Research Centre, IMDS Navi Mumbai, involving 178 patients diagnosed with Parkinson’s disease. A quasi-experimental design was employed to assess the effects of a holistic lifestyle intervention on tremor control. Participants were stratified into two cohorts: an intervention group receiving structured lifestyle modification counseling and a control group receiving standard care. Tremor severity and related clinical outcomes were systematically evaluated at baseline and at follow-up intervals of 3, 6, and 12 months post-intervention to determine the longitudinal impact of lifestyle modifications on motor symptom progression.

Results: This study demonstrated that lifestyle change counseling was beneficial, resulting in significant improvements in body tremor (from 185.5±29.7 to 145.1±18.8), between the 3rd and 12th months of follow-up, with a significant p-value of 0.001 in the intervention group. The study demonstrated strong compliance with a balanced diet, physical activity, and the cessation of tobacco and alcohol; however, adherence to meditation for stress management, regular checkups, and medication compliance was notably lower.

Conclusion: In conclusion, lifestyle modification counseling represents a viable and non-invasive approach for optimizing tremor management in individuals with PD thereby contributing to more effective tremor control in this patient population.
