The comparisons between UPDRS and MDS-UPDRS in evaluating motor dysfunctions after subthalamic nucleus deep brain stimulation

Objective: We aimed to examine whether MDS-UPDRS are able to detect longitudinal changes in motor functions after STN-DBS in comparison to UPDRS.

Background: Although MDS-UPDRS are increasingly used in the examinations of motor dysfunctions before and after subthalamic nucleus deep brain stimulation (STN-DBS) in Parkinson’s disease (PD), many researchers still use UPDRS.

Method: We examined 24 PD patients (mean age 59.2±10.6 years, mean disease duration 12.0±3.0 years) who underwent STN-DBS and whose motor functions were examined by both UPDRS and MDS-UPDRS before, 3 months after, and 1 year after STN-DBS. We evaluated the consistency between the score of Part 2 and Part 3 of UPDRS and MDS-UPDRS during off phase using Lin’s concordance coefficient and Bland-Altman’s plot.

Results: The score of Part 2 and Part 3 of both UPDRS and MDS-UPDRS significantly decreased 3 months and 1 year after STN-DBS during off phase. The score of UPDRS and MDS-UPDRS showed significant positive correlations before and after STN-DBS. We calculated estimated MDS-UPDRS score from UPDRS score using regression line and calculated Lin’s concordance coefficient (LCC) between MDS-UPDRS and estimated MDS-UPDRS. The value of LCC was 0.59~0.91 suggesting that relatively high consistency was confirmed between UPDRS and MDS-UPDRS. Bland-Altman’s plot showed that difference in both scores were basically within ± 1.96 standard deviations of the difference.

Conclusion: The utility of MDS-UPDRS in evaluating motor functions before and after STN-DBS might be equivalent to UPDRS.
